New Patient Information

Your first visit to Nathan E. Chandler, DDS establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Your inital visit will begin with new patient paperwork. We gather your personal info such as address, phone numbers, and health history etc. At this time you will also sign a consent form to treatment and agree to our office financial policies and truth in lending statement. 

Throughout the site, you will be able to view background information about our committed staff, map and directions, patient education topics, and reviews from current patients at our office. What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. During your first visit you will meet with our hygienist and they will go over your health and background information with you. You will then have x-rays taken and a prophylaxis (cleaning) performed. At the end of your appointment, Dr. Chandler will come in to greet you personally and do a final and comprehensive exam. He will also go over any treatment recommended and answer any questions you may have about a procedure. At this time you will schedule any appointments with our front office staff where any financial questions or arrangements can be made as well. We will then We look forward to your first visit.
